(no health questions). These plans are voluntary with no

Hospital
Indemnity
- Provides cash to insured when
hospitalized.
- Offset medical deductibles.
- Reimburses for certain office
visits.
Both plans cover employee and any
dependent. Both are guarantee issue. Can cover any family member
regardless if they have medical coverage OR if they are
on a different plan than you. Does not coordinate
with any other coverage.
Critical
Illness**
- Pays cash (up to $10K or $20K)
immediately upon diagnosis of heart, cancer, or other
serious/critical illness.
- Covers employee and any
dependent.
- Guarantee issue.
- Can cover any family member
regardless if they have medical coverage OR if they are
on a different plan than you.
- Multiple Lump sum cash payments
are possible within plan design.

This benefit provides a
screening that encompasses over 50 tests to provide a
thorough assessment of the individual’s health. The
screening includes:
- Full lipid, cholesterol, liver

- Complete blood count test
- Thyroid panels with TSH
- Testing of electrolytes, minerals
and glucose
- Assigned health coach…and more!
